We are excited to welcome students back to school in just a few weeks! Leading up to the first day(s) of school, there are a few important tasks that parents/guardians must complete. We have created a to-do list (below) that you can use to track when you complete each item. As always, if you need any support, please contact your school.
First Day of School:

Register for or Update Information in the Health Portal
Our school nurses use a medical record system known as the SNAP Health Portal. This web-based system provides an easy, secure, and sustainable way to update and review your child’s health and wellness information and medical documents. Having updated health and wellness records helps CPS maintain a healthy and safe environment for your child.
- New families: Please register for the SNAP Health Portal. A registration invitation was sent to unregistered users on Wednesday, August 13 from @[email protected].
Questions? Contact School Health Clinical Manager Mackenzie Henriquez at [email protected] or Immunization Coordinator Vanessa Sanz at [email protected].
- Review Immunization Requirements and Medication Guidelines
Read the Back-to-School Letter from School Nurses here for information on immunizations and medications before school starts.
Find Your Bus Information
Families should have received a notification through ParentSquare the weekend of August 15th about your child’s eligibility for a school bus, where your child’s bus stop is located, and the route.
- Use the CPS Bus Finder to find your student’s stop. Please note that some routes are still in the process of being finalized. We encourage you to check again next week to confirm.
- Download the bus tracker app for your child’s bus vendor to track their school bus by GPS. Please note that there are different bus tracking apps for each bus vendor.
Arrive at your bus stop 10 minutes early.
